Ovarian Hamartoma as An Atypical Manifestation of Tuberous Sclerosis Complex: Case Report with Diagnostic Dermatological Findings

Introduction: Tuberous sclerosis complex (TSC) is a rare genetic disease with multisystem manifestations, in which neurocutaneous symptoms usually predominate. However, ovarian lesions are unusual and may mimic malignancy. This case report was prepared in accordance with the SCARE 2025 checklist. Case report: A 43-year-old woman presented with a suspicious left ovarian mass and elevated CA-125 levels. Dermatological findings consistent with tuberous sclerosis were identified. Exploratory laparotomy revealed a benign ovarian hamartoma. Discussion: In most cases reported in the literature, a family history or neurological manifestations are present, which was not the case in this patient. Elevated CA-125 levels may occur in benign conditions, highlighting the importance of proper clinical correlation to avoid unnecessary procedures. Conclusion: It is appropriate to consider tuberous sclerosis in the evaluation of ovarian masses, especially when characteristic dermatological signs are present, even in the absence of a family history of the condition.
